This version utilizes a utility called JOTFORM. It creates the form that you see below. The form really can’t be altered in appearance, so what you see is what you get. The utility is free for basic, limited use. The student would fill out this form, and then the applicant’s chief would have to fill out a separate form to confirm permission for the student to apply.
